I voluntarily participate in this program and agree to assume all risks, including but not limited to: water-related activities, environmental hazards, wildlife encounters, and physical exertion. I hereby release and hold harmless the organization, its officers, employees, and volunteers from any and all claims, damages, or liabilities arising from my participation.
I declare that I am in good physical and mental health and am capable of participating in this program. I have no medical conditions that would prevent my safe participation. I am responsible for consulting with a physician if I have any concerns about my health.
I agree to follow all safety instructions, wear required safety equipment, and obey all directions from program leaders. I understand that failure to follow safety procedures may result in removal from the activity.
I consent to being photographed or recorded during this program and grant permission for these images to be used for educational, promotional, or archival purposes.
